The reviewer reads the manuscript and author-provided reference context, then
produces a human review and a structured issue list. The writer identifies
source mode first. In `single_file` mode, the writer edits the confirmed source
manuscript. In `chunks` mode, the writer edits source chunks and the manuscript
is reassembled afterward. The author remains responsible for the final
manuscript.
